Student-Athlete Request Form
Thank you for your interest in requesting student-athletes from The University of Alabama for your activity.
In order to process the promotional activity waiver request in a timely manner and not jeopardize a student-athlete's eligibility status, The University of Alabama reserves the right to deny any and all requests submitted less than three weeks prior to the activity/event date. Also, in the interest of our student-athlete's academic and athletic commitments, The University of Alabama reserves the right to deny any and all requests for activities/events that do not occur within a 30 mile radius of Tuscaloosa, Alabama, or those requested during the first two and last two weeks of each semester. The student-athlete is not permitted to miss class and must sign a release statement prior to the activity/event. Requests will be accepted during the summer months, however, they may be difficult to accommodate due to the time commitment of accelerated class schedules, in addition to student-athletes' summer employment, internships, and workouts.
Per NCAA bylaw 12.5.1.1 and 12.5.1.1.2: student-athletes are only permitted to participate in educational/charitable/non-profit activities/events that meet ALL of the requirements specified in the bylaws. The University of Alabama Athletics Department is required to submit written approval from the Compliance staff and Life Skills Department for all student-athletes participating in any non-profit, charitable, or educational activities/events considered incidental to participation in intercollegiate athletics.
If the sponsoring agency desires use of the student-athlete's name, picture, or appearance, the sponsoring agency must complete all necessary information regarding the activity/event on the waiver request form, including submitting proof of 501(c)(3) status if a charitable/non-profit agency.
